If you have suffered an injury on the job you are entitled by law to treatment by any practitioner you choose without a referral. What you need to have to open a case is to be sure you've reported the injury to your supervisor or personnel department. Also have date of injury, the Compensation carrier (different than your private insurer), and a breif statement of how the accident occurred.
